Ozempic Injection Site Rotation: A Simple System That Prevents Bruising
Ozempic, Wegovy, Mounjaro, and Zepbound are all injected under the skin — and all of them come with the same instruction: rotate your injection sites. Here's why that matters, a rotation system you can actually remember, and how to track it automatically.
Why injection site rotation matters
Injecting the same spot week after week can cause irritation, bruising, and lipohypertrophy — small lumps of fatty scar tissue under the skin. Scar tissue doesn't just look and feel unpleasant; it can also affect how consistently the medication absorbs. Rotating sites gives each area time to fully heal between injections.
Where you can inject Ozempic (and other GLP-1s)
All the weekly GLP-1 pens use the same three subcutaneous zones:
- Abdomen — at least two inches (5 cm) away from your belly button. The most popular site: easy to reach and see.
- Front of the thighs — the outer, upper area of either thigh.
- Back of the upper arms — easiest with help from another person.
Absorption is considered clinically equivalent across all three zones, so choose what's comfortable — the rotation is what matters.
A rotation system you'll actually remember
The simplest robust pattern is a zone-and-quadrant rotation:
- Split your abdomen into four quadrants (upper left, upper right, lower left, lower right) and add left thigh, right thigh.
- Move one position each week: abdomen UL → abdomen UR → right thigh → abdomen LL → abdomen LR → left thigh → repeat.
- Within a quadrant, inject at least an inch away from the exact previous spot.
