Novo Nordisk's official guidance for a missed weekly semaglutide dose (Ozempic and Wegovy) is:
Within 5 days of the missed dose: inject as soon as you remember, then return to your regular schedule.
More than 5 days late: skip the missed dose entirely and take your next dose on the regularly scheduled day. Never take two doses to catch up.
Why 5 days? Semaglutide has a half-life of about a week, so within that window your medication level hasn't dropped far and a late dose won't stack dangerously with the next one. Past it, injecting late would push the two doses too close together and raise the risk of nausea and other side effects.
Mounjaro and Zepbound (tirzepatide) use a slightly different window — Eli Lilly's guidance is 4 days (96 hours). Same principle: late but within the window, take it; beyond it, skip.
What to expect after a missed dose
